Our goal is to deliver comprehensive, compassionate, fair, and equitable medical care to all our patients. HOPE Center staff are multi-cultural and bilingual (English/Spanish) with trained and certified medical translators. HOPE Center has been a recognized Patient Centered Medical Home (PCMH) since 2017, offering comprehensive primary care and pain management services.
In addition to Primary Care, our medical doctors and Nurse Practitioners also specialize in delivering HIV primary care, Hepatitis C care and treatment along with being prescribers of PrEP (for HIV prevention).
HOPE Center receives specialized grant funding through NYS and NYC Department of Health (DOH) and Health Resources and Services Administration (HRSA). The funding allows HOPE Center to offer specialized medical and support services to individuals living with HIV and Hepatitis C, along with those seeking HIV preventive care through PrEP. HOPE Center also receives funding to provide community-based HIV/Hep C rapid testing and counseling services as well as linkage to care services. HOPE Center also has a NYS Medicaid funded Health Homes Care Management program which offers community-based Care Management services to qualifying Medicaid recipients.
