According to the New York State Department of Health, Yonkers is ranked highest in need of HIV-related services. The HOPE Center is committed to stemming the HIV epidemic by providing quality care to the community.

HOPE Center

Our services are provided to individuals who are at-risk for HIV infection or who are HIV infected. Services are either free or are charged to your medical insurance, including Medicaid, Medicaid HMO’s, Medicare, and most private insurance. We also have access to assist with the outpatient medical care of those with HIV who have no coverage. This coverage is through the AIDS Drug Assistance Program (ADAP) offered by the State of New York.

Our staff is multicultural and multi-lingual. We work in this field because we care! We want to help you make a difference in your health. We have the personal, professional, cultural, and life experience to help you be as healthy as you can.

We offer the following services to those infected with or affected by HIV/AIDS:

HIV Counseling, Testing, and Referral

  • Confidential rapid HIV testing for individuals who do not know their HIV status.
  • PrEP is available for people who are HIV-negative and have a high risk of being exposed to HIV through sex or drug injection.

Case Management

  • We coordinate your health care and psychosocial services to meet your individual needs, such as such as transportation, insurance coverage and housing.
  • Peer educators provide support and understandable information regarding ongoing issues impacting people with HIV and AIDS.

  • The HOPE Center’s medical and nursing staff provides compassionate and confidential medical care for adults with HIV infection.
  • Hope Center has been awarded “Best Practice in Gynecological Care for HIV Infected Women” and “Best Practice in Comprehensive Health Care for HIV-Infected Substance Users”
  • Dental services are also available to patients

Mental Health

  • Many people experience feelings of depression, anxiety, and problems sleeping when they are infected with HIV/AIDS.
  • Hope Center has a team available to diagnose and treat mental health conditions.

The program is funded through grants from the Department of Health and Human Services, New York State Department of Health (AIDS Institute) and Westchester County Department of Health.

Primary Care Medical ServicesSe habla Espanol

The HOPE Center’s medical and nursing staff provides compassionate and confidential medical care for adults with HIV infection.  Our physicians are Board Certified to treat Infectious Diseases.  Our Nurse Practitioners are credentialed as HIV Specialists.  Medical care is provided at Park Care Pavilion (2 Park Avenue, Yonkers). Because of our emphasis on easy access and patient comfort, the New York State Department of Health recognized us as a “Best Practice in Gynecological Care for HIV Infected Women” and as a “Best Practice in Comprehensive Health Care for HIV-Infected Substance Users”. Our primary care providers also diagnose and manage general medical issues such as high blood pressure, diabetes and asthma.  HOPE Center offer specialized treatment to individuals who are co-infected with Hepatitis C.  We offer many services to keep patients healthy such as vaccines, Pap tests and cholesterol screens.  If a referral to a specialist is warranted, we may refer you to one of our many on-site clinics such as Podiatry, Neurology, Cardiology, Gynecology or Ophthalmology. Dental ServicesSe habla Espanol

We offer specialized dental care to individuals living HIV. In addition to providing routine dental cleanings, extractions, x-rays and denture care, our Dentist, Dental Assistant and Dental Hygienist are trained to understand and treat the oral manifestations of HIV/AIDS. Psychiatric and Social Work Services

The HOPE Center has a psychiatrist available two days a week to diagnose and treat mental health conditions. Many people experience feelings of depression, anxiety, and problems sleeping when they are infected with HIV/AIDS. Our psychiatrist may offer medications to help with these and other problems. In addition, clients may benefit from individual and/or family counseling with one of our experienced social workers. . Case ManagementSe habla Espanol

Our social workers and case managers work with you to monitor your needs systematically and ensure continuity of care through the changing course of treatment for HIV. We coordinate your health care and psychosocial services to meet your individual needs, such as such as transportation, insurance coverage and housing. We provide short-term HIV related counseling, referrals, follow-up, crisis intervention and service plan development. . We also offer intensive community based case management services through our Community Follow-up Program (Medicaid reimbursed – COBRA case management). This program is available to anyone living with HIV who presents intensive needs and who qualifies (must have Medicaid), regardless of where they receive primary medical care. Our innovative intensive medical case management model assures that our neediest clients are supported as much as possible. This grant-funded program helps individuals identify and overcome personal, social and cultural barriers to seeking and maintaining HIV treatment. This special program is funded by the Westchester County Department of Health through a Ryan White Part A grant. Our trained peer educators work in our medical offices to answer questions from the perspective of someone who knows firsthand what you’re going through. HIV Counseling, Testing and ReferralSe habla Espanol

Quality of Care

At SJRH, ParkCare Pavilion, quality of care is a top priority. We are recognized as a Designated AIDS Center (DAC) by the New York State Department of Health. As a DAC, we offer the highest quality HIV care both for our inpatients and for those who receive care through our outpatient clinic. We constantly strive for excellence. One indication of this is our high level of client satisfaction.

Another important element of our ongoing effort to improve patient care is our participation in state and national quality improvement projects. We are active participants in both the HIVQual Project (HIV Quality of Care) of the New York State AIDS Institute, and the HAB/IHI Breakthrough Series Collaborative on Improving Care for People Infected with HIV (HIV/AIDS Bureau of US Department of Health and Human Services).

Because of our emphasis on easy access and client comfort the New York State Department of Health has recognized us as a “Best Practice In Gynecology Care for HIV Infected Women.”. NYSDOH also recognized us as “Best Practice” for Case Management and Substance Use”. If a client is admitted to St. John’s Riverside Hospital or to Park Care Pavilion Behavioral Health Services, the HOPE Center’s Discharge Planner coordinates services.
