
The cumulative incidence of reported AIDS cases in Yonkers is more than twice that for all other areas of Westchester County combined. According to the New York State Department of Health, is ranked highest in need for HIV-related services.
We have taken an important step to stem the HIV epidemic with the HOPE Center. The program is funded through grants from the Department of Health and Human Services, New York State Department of Health (AIDS Institute) and Westchester County Department of Health.
The HOPE Center wants to be a partner in your health care. Our staff is multicultural and multi-lingual. We work in this field because we care! We want to help you make a difference in your health. We have the personal, professional, cultural and life experience to help you be as healthy as you can.
Our services are provided to individuals who are at-risk for HIV infection or who are HIV infected. Services are either free or are charged to your medical insurance, Medicaid or Medicare. We also have access to coverage to assist with the outpatient medical care of those with HIV who have no coverage. This coverage is through the AIDS Drug Assistance Program (ADAP) offered by the State of New York.

Primary Care Medical Services Se habla Espanol
We offer compassionate and cutting-edge medical care for individuals with HIV infection. Our care is offered at different times and in different locations . Our team of medical doctors, nurse practitioners, registered nurses, case managers and social workers work with you to address all areas of your life and health. We also offer specialty care, including our new state-of-the-art dental office.
We integrate complementary modalities with our primary medical services. Acupuncture services are free for HIV-infected individuals who receive primary care at our medical clinics. Massage services are offered on-site for a small fee. In addition, we offer hypnotherapy services to assist with pain management and stress reduction. Case Management Se habla Espanol
Our social workers and case managers work with you to monitor your needs systematically and ensure continuity of care through the changing course of treatment for HIV. We coordinate your health care and psychosocial services to meet your individual needs. We provide short-term HIV related counseling, referrals, follow-up, crisis intervention and service plan development. Prevention and Rist Reduction Se habla Espanol
Our prevention and risk reduction services are provided to those who are HIV-infected or at-risk. We promote health practices to reduce the risk of HIV infection and harm from HIV infection. We offer support and psychoeducational groups to help those at risk avoid becoming infected. We also offer support to those already infected to maintain their health. Intensive Medical Case Management Se habla Espanol
Our new, innovative Intensive Medical Case Management Program is designed to help those individuals most in need of support in maintaining good health. This grant-funded program works to help individuals identify and overcome personal and cultural barriers preventing them from seeking HIV treatment. This program is funded by the Westchester County Department of Health through a Ryan White Title I grant. Participants in this program are part of a multi-site study of innovative techniques for maintenance in primary care, through the New York Academy of Medicine. Peer Education Program Se habla Espanol
Our trained peer educators work in our medical offices to answer questions from the perspective of someone who knows firsthand what you're going through. Peer educators provide support and understandable information on ongoing issues impacting people with HIV and AIDS. HIV Counseling, Testing and Referral Se habla Espanol
HIV counseling and testing provides persons at risk of HIV infection with prevention information relevant to their risk. We help individuals assess the risks and identify steps they are willing to take to reduce these risks. We offer the opportunity for a blood screening for the presence of antibodies to HIV. Referrals are provided to those in need of additional health, social or counseling assistance. Quality of Care
At SJRH, ParkCare Pavilion, quality of care is a top priority. We are recognized as a Designated AIDS Center (DAC) by the New York State Department of Health. As a DAC, we offer the highest quality HIV care both for our inpatients and for those who receive care through our outpatient clinic. We constantly strive for excellence. One indication of this is our high level of client satisfaction.
Another important element of our ongoing effort to improve patient care is our participation in state and national quality improvement projects. We are active participants in both the HIVQual Project (HIV Quality of Care) of the New York State AIDS Institute, and the HAB/IHI Breakthrough Series Collaborative on Improving Care for People Infected with HIV (HIV/AIDS Bureau of US Department of Health and Human Services).
Because of our emphasis on easy access and client comfort the New York State Department of Health has recognized us as a "Best Practice In Gynecology Care for HIV Infected Women."
